The issue all begins with his heart-stopping more than once in his mother’s womb.

Also, Muzan was diagnosed with a rare disease that would kill him early on in life.
There was a doctor that did everything he could to help Muzan get better.

What does the doctor use to cure Muzan Kibutsuji ?

He dosed Muzan with medicine made from Blue Spider Lily.
Unfortunately, Muzan did not recover.
Then he got frustrated by how his condition was getting worse.
Then Muzan killed his own Doctor before he could finish all the treatments.
After he killed the Doctor, he then realized the medicine was actually working.
It helped him acquire a non-human body and strength over time.
Then it made him crave human flesh as well.

Whatever the doctor had used to make the medicine with the Blue Spider Lily granted Muzan to become immortal at some point.
